The Geriatric Certificate Program (GCP) collects and retains personal information of students, graduates and other parties, including but not limited to program partners. The personal information being collected includes: name, email, mailing address, phone number, job title, and education level. This information is used for the academic, administrative, employment-related, safety and security, financial and statistical purposes of the Program, including for the administration of registration, awards, convocation, graduation communications and other fundamental activities related to being a member of the GCP community, and being a user of services provided by the GCP and its partners. The information will be used, among other things, to admit, register and process completion of the Program for students, as well as to improve program quality through anonymous GCP student surveys. Additionally, this anonymous survey information may be shared with other institutions of higher education in order to administer collaborative programs for future opportunities of continuing professional development for health care professionals. Information on admissions, registration and academic achievement may also be disclosed anonymously and used for statistical and research purposes by the GCP, other post-secondary educational institutions and the federal and provincial governments. Aside from the foregoing, the information you provide and any other information placed in a student record, or in a personnel record, will be protected and used in compliance with Ontario’s Freedom of Information and Protection of Privacy Act (RSO 1990).
